Periodontal = Gums Bleed when I brush Staging and Grading :
📏 Periodontal Examination
-
BPE:
- Upper: 3/2/3
- Lower: 4/2/3
-
Specific PPD: 47ML - 6mm PPD
-
Plaque Score: 98%
-
BOP: 86%
[BPE ](“Basic Periodontal Examination (BPE)” (“Perio Staging and Grading”, p. 1))
- Score of 3 = more in depth for that quadrant
- Score of 4 = more in depth for the entire site!
“[2018 Classification of Periodontal and Peri-Implant Diseases and Conditions](“2018 Classification of Periodontal and Peri-Implant Diseases and Conditions” (“Perio Staging and Grading”, p. 1))”
- Periodontitis ( A bit early to say this but obvious, can definitevly say after the EFP)
EFP Clinical Decision Tree
- A BPE of 3 in multiple quadrants suggests generalized CAL > 1
- We won’t really know until more in depth charting ! Which should be done before we even go down this tree
- PPD of 4 mm or more is present (although localized)
- this already makes it a periodontitis case
- Also present of gingival inflammation makes it a “Gingival Inflammation in periodontitis patient”
Clincial Steps
- OHI somewhere? todo
- Full periodontal charting
- which phase?
- After Completing periodontal charting and radiographs
- Periodontal and bone appraisal ⇒ Staging and Grading
Staging and Grading
Staging
- Interdental CAL
- won’t really know because theres recession AND pockets but lets assume its 1
- RBL
- Also won’t really know because we don’t have enough radiographs (less than coronal third from teeth we can see though )
- Tooth loss due to perio
- We need to figure out why the 14 got extracted!
- Complexity
- ==max probing depth = 6mm
Important
This is the determinant that tells us its probably stage III
- Extent and Distribution
- Localized
- for this to be true needs to be <30% of teeth involved
- BUT can also be generalized or molar incisor, we need to do the full perio chart
- Localized
Grading
- HbA1c >= 7% there for Grade C
Next steps
Now we have to move on to EXAM - Perio Treatment
