Logbook Completion Guidelines

Introduction

This document outlines the correct procedure for completing the monthly logbook, which is due at the beginning of every month. The logbook is structured into multiple sheets designed to track patient interactions, procedural progress, clinical exposure, and future planning.

Sheet 1: Patient Overview (General Clinic)

The first sheet provides an overview of all patients seen in the general clinic. Entries should reflect the status of care provided during the month.

  • Date Entry: Record the specific date a task was completed.
  • Status Indication: If a task is not yet completed, mark the status as “Ongoing.”

Example Entries

  • Patient Gillen:
    • Checkup completed on February 2nd.
    • Treatment plan approved by tutor on February 8th.
    • Conversation regarding treatment plan held on February 8th.
    • Control phase still in progress at the end of the month → Mark as Ongoing.
  • Patient Nadia:
    • Emergency extirpation performed on February 3rd.
    • Approval to complete root canal pending → Mark Treatment Plan as Ongoing.

Sheet 2: Procedure Tally

This sheet is not patient-specific. It serves as a cumulative tally of all procedures performed to track progression toward required competencies.

  • Tracking: Records the total count of specific procedures completed.
  • Auto-Calculation: The sheet automatically tallies procedures as new entries are added in later months.
  • Requirements Column: Displays the target requirements for comparison.

Example Data (February)

  • Tooth-Colored Restorations: 2 completed.
  • Gingivitis Management: Includes reassessment.
  • Extractions: Completed with supervision (e.g., with Dr. Grady).

Sheet 3: Clinical Exposure (Assisted and Observed)

This section records procedures where the student did not perform the treatment but was involved through assistance or observation. This ensures exposure is documented even if the procedure was not personally executed.

  • Entry Types:
    • Performed: Procedures done by the student.
    • Assisted: Procedures where the student assisted.
    • Observed: Procedures watched by the student.
  • Valid Contexts:
    • South Bank clinic (e.g., observing Dr. Grady).
    • Hospital Dentistry (e.g., observing Dr. Ross).
    • Extraction Clinic (e.g., assisting another student).

Example Scenario

  • Patient Nadia: Endodontics were not completed by the student; the tooth was extracted by a supervisor. The student observed the extraction. This should be recorded in this sheet to demonstrate exposure.

Sheet 4: Laboratory Log

This sheet tracks laboratory-based work.

  • Input Guidance: Hovering the mouse over the column headings provides specific instructions on what data needs to be input.
  • Status: Generally straightforward data entry based on lab tasks completed.

Sheet 5: Treatment Planning and Requirements Forecast

This sheet is used to plan expected treatments for each patient and forecast whether end-of-year requirements will be met.

  • Purpose: Allows students to check likelihood of meeting yearly targets based on current progress and planned procedures.
  • Example Plan (Patient Gill):
    • 4 Tooth-colored restorations.
    • 1 Crown.
    • 1 Single tooth endodontic treatment.
  • Progress Tracking:
    • If 45 restorations are completed by October and 4 more are planned, the student can confirm they will likely reach the target of 50 by year-end.

Sheet 6: Third-Year Presentation Case

The final sheet is designated for recording the case selected for the third-year presentation.

  • Selection: Record the case intended for presentation based on current knowledge (e.g., cases reviewed by February).
  • Flexibility: This selection is subject to change, but early recording ensures a backup choice is documented.
