    <text>## Case: Radiographic vs. Visual Detection
### Question
Have a look at this case of a child in the primary/early mixed dentition. Clinically, you cannot see any proximal carious lesions. What happens when we take bitewing radiographs?

### Findings
*   **Clinical Observation:** Visually, the proximal surfaces appear sound.
*   **Radiographic Findings:** The bitewings reveal early lesions, many of which are already approximating the dentino-enamel junction (DEJ).


*   **Reasoning:** Radiography is a vital adjunct because early proximal lesions are very difficult to visualize clinically. While radiographs are highly accurate for proximal lesions, they are less valid for occlusal lesions (detecting only about 33%).

## Case: Limitations of &quot;White Spot&quot; Terminology
### Question
Look at these three different cases. All of them present with &quot;white spots&quot; on the teeth. Are the diagnoses the same?

### Findings
*   **Case 1 (Fluorosis):** Presents as widespread, diffuse opacities involving all the teeth.


*   **Case 2 (Early Caries):** Lesions are localized to the gingival margin, associated with poor plaque control and gingivitis. These are ICDAS 2 lesions (visible without drying).


*   **Case 3 (Developmental Defects):** Demarcated opacities involving the incisal thirds of teeth 11 and 21. These are hypermineralized developmental defects of enamel.


*   **Reasoning:** The term &quot;white spot&quot; is too ambiguous. Diagnosis depends on the location, distribution, and association with plaque.

## Case: Differentiating MIH from Caries
### Question
Look at this case posted on social media, described as a &quot;very large carious lesion.&quot; Based on the pattern and location, is this primarily caries?

### Findings
*   **Location:** The breakdown is occurring on the occlusal and palatal surfaces. This is not a typical site for primary caries (which usually starts in pits/fissures or proximal contacts).


*   **Severity:** The tooth is severely broken down in a way that may not correspond to the patient's overall plaque levels or risk factors.
*   **Diagnosis:** This is an example of **Molar Incisor Hypermineralization (MIH)**. While secondary caries may be present, the primary etiology is a systemic developmental defect.

## Case: Turner Tooth (Infection-Related Defect)
### Question
Look at this erupting premolar in a six-year-old child. It shows significant breakdown and opacities immediately upon eruption. Is this caries?



### Findings
*   **Observation:** The tooth has not been in the mouth long enough for the caries process to cause this level of destruction.
*   **Diagnosis:** This is a **Turner Tooth**. It is a hypomineralized defect caused by a chronic infection of the preceding primary tooth, which disrupted the development of the underlying permanent tooth.

## Case: Comprehensive Arch Scoring (Primary Dentition)
### Question
Let's go through this case tooth-by-tooth to score for both caries (ICDAS) and developmental defects.

### Findings
*   **55:** Brown opacities present.


*   **54:** Significant breakdown involving more than half the tooth; **ICDAS 6**.


*   **53:** Breakdown on the incisal third (atypical for caries); scored as **Post-Eruptive Breakdown (PEB)** and **ICDAS 5** (due to dentine exposure).


*   **52, 51, 61, 62:** Maxillary incisors are root stumps; **ICDAS 6**.


*   **63:** Atypical defect on the incisal third; **PEB** and **ICDAS 5**.


*   **64:** Severely broken down (&gt; half tooth); **ICDAS 6**.


*   **65:** Cavitation (&lt; half tooth) with brown opacities in an atypical area; **ICDAS 5** and **Brown Opacity**.


*   **75:** Distal breakdown and brown opacities; scored as **PEB** (the most severe defect).


*   **74:** Microcavitation in pits/fissures; **ICDAS 3**.


*   **73 &amp; 83:** Well-demarcated white opacities on the incisal third; **White Demarcated Opacities**.


*   **84:** Cavitation into dentine (&gt; half tooth); **ICDAS 6**.

### Question
Score the following maxillary arch for caries and defects.

### Findings
*   **16:** Clinically sound; **ICDAS 0**.
*   **52:** Mesial lesion into dentine; **ICDAS 5**.


*   **51 &amp; 61:** Involvement of more than half the tooth; **ICDAS 6**.


*   **62:** Mesial and palatal involvement (&gt; half tooth); **ICDAS 6**.
*   **65:** Mesial cavity into dentine (**ICDAS 5**) and a gray shadow on the palatal aspect (**ICDAS 4**).

## Case: Lower Arch Scoring (ICDAS 2 vs 3)
### Question
Look at the 36 and the 35 in this image. How should they be scored?

### Findings
*   **36:** Shows changes in the pit and fissure system slightly beyond the fissure itself, but no cavitation; **ICDAS 2**.
*   **35:** Shows microcavitation in the enamel; **ICDAS 3**.
*   **Reasoning:** If you are in doubt between a gray shadow (ICDAS 4) and microcavitation, always score lower (**ICDAS 3**) to avoid over-treatment.

## Case: HSPM and PEB
### Question
Look at the 75 and 85 in this case. What do you see?

### Findings

## Case: Identifying Non-Carious Loss
### Question
Look at this frontal image. There are several lesions visible. What should be excluded from caries scoring?

### Findings
*   **Observation:** There is visible **attrition** (non-carious tooth surface loss).
*   **Reasoning:** Attrition and general tooth wear should not be scored as caries or developmental defects.
*   **Scorable Lesions:** The 33 buccal surface shows cavitation well into dentine; **ICDAS 5**.

## Case: Upper Arch (Gray Shadows and Cavities)
### Question
Evaluate the 54, 64, and 65 in this maxillary view.

### Findings
