Bridges are becoming less common due to implants!

Case: Middle aged man with bone loss

  • Not enough space for implants
  • doesn’t want to do ortho because he’s old
  • Often the adjacent canine is sufficient to cantilever off
    • However the 12 would be tiny because of the space available
    • so they opted to prepare the 11 and 13
    • by preparing both teeth the lab has more control of the pontic

Sinus extends too far, would need bone graft or sinus lift!

Splinting Considerations

  • Although mentioned in books, and very useful in perio, patient oral hygiene status should be carefully considered!

Contra Indications

  • Poor periostatus of abutments -absence o distal abutment in posterior sections
  • long span bridges -poor OH -Extremely tilted teeth

Components

Path of Insertion

  • All teeth need to have the same Path of Insertion
  • Think about it like a massive undercut

Case of Extremely tilted teeth

  • You can try and chang ethe path of insertion to match the other teeth in the area
    • but you need to consider all the other biolgical problems

Case: Mesially tilted lower molars

Orthodontics to selectively upright the tooth

Partial preparation

Pontics

  • Can be classified as those who make mucosal contact and those who don’t

Saddle/ridgelap

  • Looks good
  • Concave fitting surface is not cleansable -thus violates the biological design and is contraindicated in all cirumcstances

Modified Ridge lap

Lingual modification to open the cervical embrasues

  • helps to facilitate hygiene!

Conical Pontic

  • bullet/egg shap
  • convex with only one point of contact -not suitable for broad residual ridges due to food trapping

Ovate Pontic

Sanitary/hygienic

Not really done much anymore, but used for cleaning
